The documentation needed in the application forms relies upon the training provided by Fellows of the Academy. You will be asked to fill in forms and upload required documents for a complete application as well as pay the application fee through the site. The step-by-step process allows you to fill out your application at your own pace, save your work, and edit the application until you are ready to submit your work.

The Academy suggests creating an account and becoming familiar with SM Apply well in advance of deadlines. Create an account even if you are in the beginning of the process for access to the SM Apply section.

Resources Available in SurveyMonkey Apply

Once you create an account in SM Apply and log in, the Resources are located at the top of the page. We suggest you download the documents that relate to your application. Please use chrome as the search engine for best results. You do not need a box.com account to download files.

May 2023 Rubric Clarification Update

The Certifying Committee has recently updated the rubric for the student profile and application lessons to provide clarity of the expectations when submitting an application. The standards have stayed the same, but the rubric has more detailed descriptions of each item.  The hope is that Fellows, FITs, trainees, and applicants will find these refreshed rubrics clear and helpful.

However, there is one notable difference in the rubrics; the student profile is separated by the levels (Associate, Certified, Fellow), including a comparison to delineate the differences between the standards of each level reflected in the Curriculum Handbook & Fellow Handbook.

The Rubrics go into effect for the November 10, 2023 application deadline.

July 2023 Fellow Rubric Updates for Video and Syllabus

The Certifying Committee made clarifications on the Video Rubric and is introducing the Syllabus Rubric to provide more explicit guidelines for assessing and evaluating submissions.

The Rubrics go into effect for the November 10, 2023 application deadline.

It’s best to use Chrome as the web browser to access the box files.
